 NOTE: ALL HORSES MUST BE BLOODTYPED BEFORE A TRANSFER CAN BE COMPLETED!

Complete the form below, send to the M.P.H.A. with the Horse’s Registration Papers and the $25.00  transfer fee. (You DO NOT need to sign the back of the papers. The M.P.H.A. Registrar does that.) Mail To
